In most cases, we will only treat patients who are referred by a doctor. If you have not yet seen a doctor but feel you could benefit from Physical Therapy, call or visit your doctor and ask for a prescription for Physical Therapy (as well as an insurance referral if your insurance requires one). We will work closely with your doctor to provide a comprehensive treatment plan.
Call the clinic most convenient to your location. The receptionist will schedule your initial appointment within 24–48 hours — same-day appointments are often available. If you need to cancel, please provide 24 hours notice. Bring your insurance information, doctor's prescription, and insurance referral (if applicable) to your first visit.
Bring your insurance card, doctor's prescription and referral (if required), and wear comfortable clothes you can move in. Your therapist will ask about the nature and history of your pain, perform a physical evaluation, and create a specialized treatment plan — which will begin at that first visit. Plan for about 1 hour and arrive 10–15 minutes early to complete paperwork.
We participate with most major carriers including Aetna, APA Partners Health, Blue Shield of NENY, CDPHP, Empire BC/BS, Blue Choice, Healthnet, Empire Plan/United Healthcare, GHI/GHI HMO, Medicare, MVP, No Fault, Wellcare, and Workers' Compensation. We bill your insurance directly. If your plan requires a co-pay, you are responsible for that amount at each visit. Private-pay rates and payment plans are also available for uninsured patients.
A prescription is a written order from your doctor for Physical Therapy. It must include your name, diagnosis, the doctor's signature, and the date. Many doctors also specify the number of visits. You must bring this to your first visit.

An insurance referral is a separate form required by some insurance plans. Your doctor's office provides it — you can bring it to your first visit or have it faxed directly to us.
